Source disclosure

Each receipt has a source disclosure switch, separate from the cryptographic claim. Private: count only, with the line Source identities withheld by publisher. Names: source names without locations. Full: a public source ledger of names, types, dates and roles. This tester build shows one receipt in each mode.
